New Patient Info
Please bring to your visit:
- Photo ID and all insurance cards
- Any prior medical records and completed registration form
- Full amount of payment for your co-pay, deductible, or co-insurance as this will be collected at the visit (Insurance contracts require you to pay this at the time of your visit to the doctor). MasterCard, Visa, American Express, debit cards, personal checks, cash all accepted. There is a fee for returned checks.
More Information About Your Appointment Time
Your appointment time is the Check-In Time (not the time you see the doctor). Each visit can last up to 2 hours if testing and treatment is performed at the same visit to save you a trip back to the office on another day.
Please call us if you have questions or need to re-schedule/cancel an appointment. No-shows limit the ability for Dr. Voo to provide care for someone else.
